Phalaenopsis care observation checklist

Take one page of notes before changing your orchid’s care. A review date reminds you to observe; it is not a watering instruction.

Print the sheet and write your observations by hand. The downloadable PDF is a one-page A4 worksheet; it has no fillable form fields and does not save answers online.

Plant label or name:

Observation date and last flowering period, if known:

  1. Confirm the plant is a Phalaenopsis. If unsure, photograph the label and the whole plant.
  2. Photograph visible roots, leaves and any flower spike. Date the photographs.
  3. Note the pot, drainage holes and potting material. Write “unknown” where needed.
  4. Check moisture below the surface. Record how you checked it.
  5. Record the plant’s location, light and any recent move.
  6. Record watering, fertilizer, repotting and other recent changes.
  7. Write down the main concern and the source you used to choose a next step.
  8. Set a date to observe again. Water only after checking the plant and mix.
